Retreat Registration

In the event of an emergency, I authorize Faithful Church of God, Inc., its designated leaders, volunteers, or representatives to contact emergency services and arrange appropriate emergency medical attention for the participant when I cannot be reached in a timely manner. I understand that every effort will be made to contact the emergency contact or parent/legal guardian as soon as reasonably possible.

I understand that participation in the Faithful Church of God, Inc. retreat may involve travel, lodging, meals, outdoor activities, recreational activities, transportation, and other group activities that may carry ordinary and unforeseen risks. I voluntarily choose to participate and accept responsibility for the risks associated with participation. To the extent permitted by applicable law, I agree to release and hold harmless Faithful Church of God, Inc., its pastors, officers, leaders, employees, volunteers, and authorized representatives from claims for injury, illness, loss, damage, or expenses arising from my participation in the retreat, except to the extent caused by conduct that cannot legally be released or waived. I understand that the church cannot guarantee that accidents, injuries, illnesses, property loss, or other incidents will not occur during the retreat. I also understand that my personal health or automobile insurance is primarily responsible for eligible medical or related expenses incurred during the retreat.

I certify that I am the parent or legal guardian of the minor named above. I give permission for my child to participate in the Faithful Church of God, Inc. Retreat. I have provided accurate emergency, medical, and insurance information and understand the terms of this permission and release form.

I confirm that the information provided on this form is accurate and complete. I agree to follow the rules, instructions, safety requirements, and reasonable directions of Faithful Church of God, Inc. and its retreat leaders.
